Todd County AXIS Program
We strive to serve youth through direct intervention at the family-school-community interface through professional assessment, service planning, monitoring, and evaluation.
Our Goals
The hope in developing this program is that children and families have better access to resources within our community, especially those related to mental health needs.
Who are we?
We provide FREE case management services to all students in the Todd County School District.
AXIS case managers work to better connect students and their families to services within the community.
All youth/families that we work with may not need the same services. Some may only need short-term assistance, and some may need longer-term assistance.
What is our goal?
Our goal is always to meet youth and families where they are and to support families in choosing the best fit for their needs.
How Can We Help?
• Basic Needs: Nutrition, Clothing, Toiletries, Daily Activities
• Housing: Living Situation, Environment
• Transportation: Availability, Dependability, Sustainability
• Financial Resources: Employment, Income, Savings
• Medical Care: Status, Needs
• Mental Health: History, Risk, Needs, Treatment
• Addiction / Substance Use: History, Risk, Treatment
• Educational: Attendance, Grades, Behavioral Referrals
• Support System: Accessibility, Reliability
How does it work?
Our program is completely voluntary and operates from referrals made by parents, teachers, staff, and outside agencies.
After the referral, an AXIS case manager will reach out to a parent/guardian to gain consent, gather information and form an individual plan relevant to the need(s).
We are available to assist in setting appointments and developing plans for outside services.
What about mental health services?
Students have access to school-based counseling and therapy through us or outside community agencies.
A knowledgeable case manager will walk you through the options and set up a referral if needed.
